Thank you for your interest in supporting BC Women’s Health Foundation. If you or your organization wishes to host a fundraising campaign or event to benefit BC Women’s Hospital + Health Centre and Women’s Health Research Institute, we kindly request you to submit this completed proposal to the Foundation for support and approval.

    Please note that 15% of all donations will be allocated to the Foundation’s Most Urgent Needs fund. This can include funding essential equipment or programs at BC Women's Hospital + Health Centre, Women's Health Research Institute, as well as the work of the Foundation.

    Do you plan to use the BC Women’s Health Foundation’s name + logo in your event/campaign promotions?

    By selecting YES you acknowledge that all materials (printed or digital) mentioning the Foundation or containing the Foundation logo
    must be submitted for approval prior to publishing/printing. Logo usage will be valid for one year from end of event/campaign.

    Community Partnership AGREEMENT

    As a Community Partner I agree to:

    • Portray a positive, credible public image on behalf of BC Women’s Health Foundation while conducting all activities related to this fundraising event/campaign.

    • Abide by the Canada Revenue Agency (CRA) guidelines regarding the issuance of charitable tax receipts. (BCWHF will provide all necessary information & assist you with the process.)

    • Obtain all necessary permits, licenses or insurance. Including but not limited to, Gaming License, Liquor License, Event Insurance.

    • Obtain authorization and approval from BC Women’s Health Foundation for the use of its name and logo in any and all media and print materials related to this event or campaign. Logo usage must be done in accordance with our guidelines.

    • Handle any monetary transactions, and present the proceeds to BC Women’s Health Foundation within 30 days following the event or campaign.

    • Submit the complete name, addresses and donation information to BC Women’s Health Foundation so that tax receipts may be issued. (Without this complete information, receipts will not be issued.)

    • Organize the event/campaign to its entirety; including but not limited to, production, marketing, ticketing, staffing, and/or volunteers for the event.

    • Incur any associated costs for the event/campaign.

    • Use my own mailing list for the event.

    • Notify BC Women’s Health Foundation prior to the original event day if the event is cancelled.

    • Receive final approval and confirmation of event proposal from the Foundation before publicizing our connection to the event.

    • Adhere to BC Women's Health Foundation's gift acceptance and processing policy.

    BC Women's Health Foundation:

    • Agrees to recognize the event/campaign in accordance with its donor recognition policies.

    • Agrees to provide a charitable receipt for all gifts of $50+ received by the Foundation in accordance with the Canada Revenue Agency guidelines.

    • Agrees to provide support and guidance to event/campaign organizers where required.

    • Shall not incur any cost or liability associated with this event/campaign.

    • Reserves the right to withdraw the use of its name and logo at any time.
